SURG-02. INTRAOPERATIVE RADIOTHERAPY FOR BRAIN METASTASES: EVALUATION OF ELECTROCORTICOGRAPHY AND PERI-OPERATIVE SEIZURE RISK

Abstract BACKGROUND Intraoperative radiotherapy (IORT) has emerged as a viable tool for consolidation of surgery and radiation brain metastases into single episode care, but its relative novelty compared to SRS or fractionated warrants evaluation safety. While post-operative seizure risk is typically minimal with the use anti-epileptic drugs during peri-operative phase, intra-operative epileptiform discharge remains unknown. METHODS Five consecutive cases surgically resected treated IORT underwent electrocorticography via direct cortical electrode recordings adjacent resection cavity paddle electrodes used mapping phase reversal. Continuous were made before, during, after delivery analyzed evidence radiation-associated changes. RESULTS No significant electrophysiological change was noted in any cases, nor there clinical immediate period. These data represent first human subjects radiation. CONCLUSIONS In this small cohort metastases, does not appear impact activity precipitate seizures. Further studies are needed determine long-term incidence treatment associated changes if they exist.
